Importance of Historical Data

For information of my colleagues, beeare that in QMS, history is very important. If you do not keep records of sufficient length of time, with enough safety factor, 2 or 3 times, you will be deemed as low quality. Because no CQI. For doctors, I avoid doctors who do not keep a complete historical record of my case and medicine. Those doctors who keep manual records are suspect. Which is why I prefer doctors, that have online medical records. Like Dr. Mansor, Sandakan. Or hospitals like SMC. Unfortunately, when SMC was taken over by KPG, my records were not transferred. Dr. Daniel Putatan kept a manual file that he can flip through. After 20 years, the writings are not clear and other doctors have problem reading. Some new doctors do not even bother to look up at the records. I was shocked, and scared to see the new doctors from govn hospitals. I don't think I am alone. That is why new doctors have problems setting up their own clinics.
